We setup a lifestyle screening and analytics service that is specific to your organization at a custom URL at our screening website, your constituents enter a Service Access Code to access the screening service at your URL

Data generated by lifestyle reports is automatically sent in real-time to your data analytics dashboard across 25 lifestyle topics, organized  by genders, age groups and showing aggregate population Lifestyle Risk Scores by answer response

Report Page Branding

The report page of your screening service can be branded to your organization “This Memory Protection Report is provided courtesy of Abc Co.”

Custom Fields

Talk to our sales office if you need custom fields added to your screening service, for example additional ways to segment your audience, or a different structure for our  demographic questions

Report Lookups

Each lifestyle report has a Unique I.D. Number, customer-facing healthcare professionals and health coaches can record these Report ID’s by client or patient and look them up again at any time with our included Report Lookup service

Service Setup Process and Fees

We charge a total Service Setup Fee of $7,500 which includes service establishment, data analytic dashboards setup, custom fields setup, report page branding and all software licenses and custom software code required to operate the service

When you register with us you pay a 50% deposit ($3,750) at which time we setup your environment for you to test, this process includes liaison with your Account Administrator on specific setup details

Once you’re happy that things are working the way you expect, the balance payment of 50% is made at which time we add 1,000 included lifestyle screenings and reports to your service and dashboard to get you started

Service Fee

We charge a monthly service fee of $495 to maintain your service which includes daily backups of your service environment and all of your data, updates to all software that powers the service, and client support 7 days a week by SMS, phone or email

Monthly service fees commence 30 days following your final payment date for service setup

Screenings And Reports

After your initial 1,000 included reports, screenings/reports are ordered by you in advance by ordering Report Credits (one Report Credit = one lifestyle screening and report)

Add additional Report Credits to your account at any time (minimum purchase 1,000), they appear in your Reporting Dashboard and show your report threshold, and how many credits you have remaining

Report Credits are charged at $4 per credit to our commercial clients, we provide a 50% discount to health advocacy organizations, non-profits, and to military veteran organizations

Purchase Data Outcomes

Clients wishing to conduct deep analysis across the lifestyles of their customers can purchase a copy of their response data for $10 per report, data is provided to you in Excel CSV format

Your data file includes responses to 25 lifestyle questions, 25 lifestyle risk ratings, a Total Lifestyle Risk Score, answers to 4 demographic questions, 42 cognitive decline symptoms, 16 chronic health outcomes and 5 dementia outcomes, and responses to any custom fields

Lifestyle risk scoring allows you to conduct complex risk analysis across your population by lifestyle topic, health outcomes, demographics and custom fields, data pricing includes any Q&A liaison that your data science team needs with us on an ongoing basis
