We setup a lifestyle screening and analytics service that is specific to your organization at a custom URL on our screening website

Your constituents use a Service Access Code to take a lifestyle assessment and get an instant online personal Memory Protection Report with access to our online learning centers

Data generated by assessments is automatically added in real-time to your data analytics dashboard across 25 lifestyle topics, it’s organized  by genders and age groups and includes aggregate Community Risk Scores for each answer response

Report Page Branding

The report page of your screening service can be branded to your organization “This Memory Protection Report is provided courtesy of Abc Co.”

Custom Fields

Talk to our sales office if you need custom fields added to your screening service, for example additional ways to segment your audience, or a different structure for our demographic question fields

Report Lookups

Lifestyle reports are HIPPA-compliant, generated at encrypted URL's, and maintain the privacy of the respondent

Each lifestyle report is stored permanently on our servers at an encrypted URL under a Unique I.D. Number

View report summaries by individual participant at any time using our included Report Lookup service

Service Setup Process and Fees

We charge a total Service Setup Fee of $7,500 which includes service establishment at your custom URL, data analytic dashboards setup, custom fields setup, report page branding, and all software licenses and custom software code required to operate the service

When you register for service you pay a 50% deposit ($3,750) at which time we setup your environment for you to test; this process includes liaison with your Account Administrator on specific setup details

Once you’re happy that things are working the way you expect, the balance payment of 50% is made at which time we add 1,000 included lifestyle screenings and reports to get you started

Service Fee

We charge a monthly service fee of $495 to maintain your service which includes daily backups of your service environment and all of your data, your analytics dashboards, regular updates to software that powers the service, and our white glove client support service which is available 7 days a week by SMS, Zoom, phone or email

Monthly service fees commence 30 days following your balance payment for the service setup

Scaling Your Service Usage

After your initial 1,000 included reports, additional screenings/reports are ordered by you in advance with Report Credits (one Report Credit = one lifestyle screening and report)

Add Report Credits to your account at any time (minimum purchase 1,000), they appear in your Reporting Dashboard and show your report threshold and how many credits are remaining; everything updates in real-time as reports are completed

Report Credits are charged at $4 per credit, we provide a 50% discount to health advocacy organizations, non-profits, and military veteran organizations

Clients that want to conduct deep analysis on the lifestyles and self-reported health outcomes of their customers can purchase a copy of their response data for $10 per report, data is provided to you in Excel CSV format and includes;
